Breaking new ground in the realm of cardiology in India, a remarkable feat has been achieved by the ingenious Dr. Ravinder Singh Rao and his exceptional team. At Lilavati Hospital in Mumbai, they successfully performed India’s first-ever LAMPOON-TMVR procedure on a 76-year-old cardiac patient who had previously undergone open heart surgery.

The LAMPOON (Laceration of the Anterior Mitral Leaflet to Prevent Outflow Obstruction) procedure stands as a transcatheter electrosurgical technique that involves splitting the anterior mitral valve leaflet just moments before a Transcatheter Mitral Valve Replacement (TMVR) takes place. This innovative approach opens up new possibilities for cardiac patients who have already undergone prior open heart surgery and face high risks associated with subsequent surgical interventions.

Dr. Ravinder Singh Rao sheds light on the significance of the LAMPOON procedure, explaining that mitral valve stenosis, which restricts blood flow into the heart’s main pumping chamber, often necessitates TMVR as a highly effective and minimally invasive treatment for elderly patients with degenerated mitral valves. However, complications can arise when the heart leaflet obstructs blood flow, posing a life-threatening situation known as Left Ventricular Outflow Tract (LVOT) obstruction. By employing electrosurgery and delicately cutting the leaflet with an electrified wire, the team overcomes this challenge. Success in such a complex procedure hinges upon the experience and expertise of the team, which included Dr. Anand Rao, Interventional Cardiologist, Dr. Namrata Kothari, Cardiac Anesthetist, and Dr. O.H. Jaiswal, Cardiac Surgeon.

During the groundbreaking LAMPOON procedure, two catheters are skillfully inserted through the patient’s groin area, navigating the intricate network of blood vessels until they reach the patient’s heart. Employing an electrified wire threaded through the catheter, the team deftly splits open the obstructing leaflet. With this hurdle overcome, the patient can then proceed with the transformative TMVR procedure.

Dr. Ravinder Singh Rao emphasizes the significance of this achievement in the context of the elderly patient’s medical history, which includes a Mitral Valve Replacement surgery in 2008, a right submandibular salivary gland excision in 2015, a gallstone pancreatitis admission in 2020, and a Laparoscopic Cholecystectomy in the same year. The LAMPOON procedure became a crucial turning point, replacing the previous valve with a new one.

With its capacity to reduce mortality risks for high-risk senior citizens aged 65 years and above, the LAMPOON procedure emerges as a true boon. Additionally, the hospitalization period is significantly minimized, with patients typically spending only one day in the intensive care unit (ICU) and approximately four days in the ward for continuous monitoring.

Lt. Gen. (Dr.) V. Ravishankar, VSM (Retd.), COO, and Cardiac Surgeon at Lilavati Hospital, expressed immense pride in the accomplishment, highlighting the hospital’s commitment to ensuring safe outcomes for routine transcatheter aortic and mitral valve replacement procedures. The experienced team at Lilavati Hospital stands ready to provide exceptional care to patients, paving the way for further advancements in the field of cardiology.
